The Fibroscan technology is used to detect liver fat accumulation and fibrosis. It employs the same principles as sonar and ultrasound, emitting high-frequency sound waves into the liver tissue and measuring the reflected waves in decibels per meter (dB/m). The presence of more fat in the liver results in faster sound wave reflection compared to liver tissue with less fat. Similarly, liver stiffness is measured, where a stiffer liver allows sound waves to pass through more quickly. This method of examination does not cause discomfort or carry the risk of complications associated with traditional methods involving liver puncture.

The Urease Breath Test is a non-invasive method for detecting Helicobacter pylori infection, a major cause of gastric and duodenal ulcers, and gastric cancer. This test can both detect the bacteria and confirm the successful outcome of treatment.

โรคข้อและรูมาติสซั่ม

Normally, humans have two kidneys, which can function as backups for each other. เอกซเรย์ทั่วไป(General  x-ray) เป็นการตรวจเอกซเรย์ที่ทุกท่านคุ้นเคยกันดี เช่น เอกซเรย์ปอด เอกซเรย์กระดูกแขน,ขา เอกซเรย์กระดูกสันหลัง เอกซเรย์ศีรษะ เป็นต้น
เอกซเรย์คอมพิวเตอร์(Computerized Tomography)  เป็นการตรวจทางด้านรังสีวินิจฉัยอย่างหนึ่ง โดยใช้เอกซเรย์หมุนรอบตัวผู้ป่วย เก็บข้อมูลในรูปแบบ 3 มิติ แล้วใช้ระบบคอมพิวเตอร์นำข้อมูลที่ได้สร้างเป็นภาพในแนวตัดขวาง และภาพในแนวระนาบอื่นๆ รวมทั้งภาพ 3 มิติ ทำให้เห็นอวัยวะภายในได้อย่างชัดเจน สามารถตรวจส่วนต่างๆในร่างกายได้เช่น

เอกซเรย์เต้านม เป็นการตรวจโดยใช้เอกซเรย์พลังงาน่ำถ่ายภาพของเต้านม เพื่อใช้ในการตรวจินิจฉัย หาความผิดปกติของเต้านมตั้งแต่ระยะเริ่มแรก ซึ่งสามารถตรวจได้ทั้งเมื่อมีอาการและยังไม่มีอาการผิดปกติ จากการวิจัยพบว่าการตรวจแมมโมแกรมเป็นประจำทุกปีทำให้สามารถตรวจหามะเร็งระยะเริ่มแรกได้ ซึ่งทำให้มีโอกาสสูงในการรักษาโรคให้หายขาดได้ โดยไม่ต้องตัดเต้านมทิ้งทั้งเต้า

2. Computerized Tomography (CT Scan):

การผ่าตัดต่อนิ้ว (Digital replantation)

เป็นการผ่าตัดโดยการใช้กล้องจุลทรรศน์มาใช้ในการต่อเส้นเลือดและเส้นประสาท เทคนิคการผ่าตัดต่อนิ้วได้ถูกพัฒนาขึ้นเรื่อยๆ จนมีผลการรักษาที่ดีขึ้นจนถึงปัจจุบัน เมื่อเกิดการบาดเจ็บ ที่ทำให้นิ้วมือขาด ผู้ป่วยหรือผู้ที่นำส่งมาที่โรงพยาบาลควรนำชิ้นส่วนต่างๆ  ที่ขาดมาให้ครบ ก่อนมาโรงพยาบาลให้ล้างชิ้นส่วนที่ขาดด้วยน้ำสะอาด แล้วนำผ้าสะอาดชุบน้ำพอหมาดๆ ห่อชิ้นส่วนนิ้วไว้ ใส่ในถุงพลาสติก ปิดปากถุงให้แน่น  แล้วนำไปแช่น้ำแข็ง (ไม่ควรให้ชิ้นส่วนสัมผัสกับน้ำแข็งโดยตรง) ในส่วนของตอนิ้ว (มือของผู้ป่วย) ให้ใช้ผ้าพันปิดแผลแน่นๆ เพื่อให้เลือดหยุดไหล ไม่ควรใช้วิธีการขันชะเนาะ เพราะจะทำให้เส้นเลือดบาดเจ็บได้ จากนั้นรีบนำผู้ป่วยส่งโรงพยาบาลให้เร็วที่สุด เนื่องจากหากทิ้งระยะเวลานาน  เนื้อเยื่ออาจขาดเลือดและตายอย่างถาวรได้ ทำให้ไม่สามารถต่อนิ้วได้สำเร็จ​และเมื่อมาถึงโรงพยาบาล

แพทย์จะพิจารณาผ่าตัดต่อนิ้วให้กับผู้ป่วยทีได้รับบาดเจ็บดังต่อไปนี้ คือ

 

• นิ้วโป้งขาด
• นิ้วขาดหลายนิ้ว
• นิ้วขาดในเด็ก
• นิ้วขาดในตำแหน่งที่อยู่ปลายต่อข้อกลางนิ้วมือ

​แต่ถ้าหากผู้ป่วยมีภาวะดังต่อไปนี้ แพทย์จะพิจารณาทำการผ่าตัดรูปแบบอื่นแทน ได้แก่

1. นิ้วที่ขาด มีภาวะเนื้อเยื่อบาดเจ็บรุนแรงมาก แพทย์พิจารณาแล้วว่าโอกาสต่อนิ้วสำเร็จค่อนข้างต่ำมาก
2. นิ้วขาดเป็นหลายท่อน ในนิ้วเดียวกัน
3. ผู้ป่วยมีโรคประจำตัว หรือภาวะการบาดเจ็บร่วมอื่นๆ ที่ทำให้ผู้ป่วยไม่ปลอดภัยต่อการผ่าตัดที่ต้องใช้ระยะเวลานาน
4. ผู้ป่วยที่มีโรคหลอดเลือดไม่ดี
5. ผู้ป่วยที่มีภาวะทางจิตไม่ปกติ ทำให้ไม่สามารถปฏิบัติตามแผนการรักษาอย่างเคร่งครัดได้
6. ผู้ป่วยที่ถูกส่งตัวมาโรงพยาบาลช้าเกินไป ทำให้เนื้อเยื่อของนิ้วมือตาย ไม่สามารถทำการต่อให้สำเร็จได้

สำหรับวิธีการผ่าตัดต่อนิ้วนั้น ใช้วิธีการดมยาสลบ ผู้ป่วยจะไม่รู้สึกตัวขณะผ่าตัด แพทย์จะทำการตัดกระดูกให้สั้นลง เพื่อให้สามารถเย็บต่อโครงสร้างอื่นๆได้โดยไม่ตึงเกินไป จากนั้นจะเย็บเส้นเอ็นที่ใช้ในการเหยียดและงอนิ้วมือ แล้วทำการต่อเส้นประสาท เส้นเลือดดำและเส้นเลือดแดงโดยใช้กล้องจุลทรรศน์ตามลำดับ

หลังการผ่าตัด ผู้ป่วยต้องนอนพักฟื้นที่โรงพยาบาลเป็นระยะเวลา 5-7 วัน เพื่อให้ยาฆ่าเชื้อ รอแผลติด และเฝ้าระวังภาวะต่างๆ ที่ทำให้เลือดไม่ไปเลี้ยงปลายนิ้ว ได้แก่ภาวะหลอดเลือดหดเกร็ง เลือดดำคั่ง    ลิ่มเลือดอุดตันในหลอดเลือดแดง ซึ่ง 2 ภาวะแรกนี้ สามารถทำการรักษาที่เตียงผู้ป่วยได้ ไม่ต้องเข้าห้องผ่าตัด แต่ถ้าหากเกิดลิ่มเลือดอุดตันในหลอดเลือดแดงขึ้นแล้ว ต้องนำผู้ป่วยเข้าห้องผ่าตัดเพื่อทำการตัดต่อเส้นเลือดใหม่

ในขณะที่นอนอยู่ในโรงพยาบาล ผู้ป่วยควรหลีกเลี่ยงปัจจัยกระตุ้นที่ทำให้หลอดเลือดหดตัว ซึ่งจะทำให้นิ้วขาดเลือดได้ ได้แก่ การดื่มชา กาแฟ การสูบบุหรี่ การอยู่ในที่ที่อากาศเย็นๆ อาการตื่นเต้นตกใจ

​         เมื่อนิ้วไม่มีสัญญาณที่บ่งบอกถึงอาการขาดเลือด และแผลเริ่มแห้งดีแล้ว แพทย์จะอนุญาตให้ผู้ป่วยกลับบ้านได้ และจะนัดผู้ป่วยมาตรวจติดตามดูอาการ จนกระทั่งกระดูกนิ้วติดดี แล้วจึงเริ่มการทำกายภาพบำบัดเพื่อให้ผู้ป่วยสามารถขยับนิ้วได้ดีขึ้น

Digital replantation surgery involves using a microscope to reconnect blood vessels and nerves. This technique has evolved over time, leading to improved treatment outcomes. When an injury results in the loss of a finger, the patient or the person accompanying them to the hospital should bring all the missing parts. Before arriving at the hospital, the missing parts should be cleaned with clean water and wrapped in clean cloth soaked in clean water. Then, the wrapped parts should be placed in a plastic bag, sealed tightly, and immersed in ice water (without direct contact between the missing parts and the ice water).

For the injured finger (the patient's hand), a clean cloth should be tightly wrapped around the wound to stop bleeding. Avoid using tourniquets as they can further damage blood vessels. Then, the patient should be quickly transported to the hospital because delaying treatment can lead to permanent tissue damage, making successful finger reattachment impossible.

Doctors consider finger reattachment surgery for patients who have suffered the following injuries:

• Complete loss of the thumb.
• Loss of multiple fingers.
• Finger loss in children.
• Finger loss at the fingertip or near the middle joint.

However, if the patient has the following conditions, doctors may consider alternative surgical approaches:

• Severe tissue damage in the missing finger(s), making successful reattachment unlikely.
• Multiple segments missing from the same finger.
• Underlying medical conditions or additional injuries that make surgery risky or prolonged.
• Poor blood vessel condition.
• Psychological conditions that prevent compliance with postoperative care.
• Patients are brought to the hospital too late, the tissues of the finger may have died due to insufficient blood supply. In such cases, it may not be possible to successfully reattach the finger.

During the replantation surgery, the patient is under general anesthesia, so they do not feel anything. The bone is trimmed to allow for easier stitching without excessive tension. Then, tendons, veins, and arteries are stitched in sequence using a microscope.

After the surgery, the patient needs to stay in the hospital for 5-7 days for observation, wound care, and to ensure proper blood circulation to the reattached finger. If there are signs of reduced blood flow or complications, additional surgery may be necessary.

During the hospital stay, patients should avoid activities that could decrease blood flow to the finger, such as drinking tea, coffee, smoking, or being in cold environments. Once the finger shows signs of good blood circulation and the wound begins to heal, patients can be discharged with follow-up appointments for further evaluation and rehabilitation to regain finger movement.
